During your care, the financial issues you may face can be very complex. The following information will help you ask the right questions and contact the appropriate resources for assistance.

If you have Insurance Coverage

Your best source of information about your specific coverage is the insurance company that issued your health insurance policy or your employer's benefit office, if you are covered under a group policy through work.

Questions to ask:

Is HOA a covered provider?  If your plan does not cover our office visit, ask if there are any exceptions. If no exceptions exist, you may still obtain care from our office, but it will be at your own cost.
What is the coverage for inpatient and outpatient services, medications, prescriptions and diagnostic procedures?
Are there any limitations or restrictions on cancer care?
Are there any out-of-pocket expenses?

Tips:

Ask for clarification if you don't understand anything that is said to you.
Be sure to follow all guidelines and fill out all paperwork requested by your insurance company.
Bring all necessary forms and referrals to your appointment.
Always keep a written record of the organization you contacted, their phone number, the name of the representative and a brief note of what they told you.

If you have NO Insurance Coverage or Insufficient Coverage

Many people do not have full insurance coverage and some have no coverage at all. If this is the case, there are two main options: deferred payment plans, and/or other sources of financial assistance depending on your ability to meet eligibility requirements.

If you require assistance, please notify your health care team. The team will then contact the patient advocate to help you determine the resources available to you.

If you need assistance with your prescriptions, there is assistance available to alleviate the plight of an ever increasing number of patients who cannot afford their prescription medication.

You will receive a bill from HOA Cancer Center.

Your representative is available to discuss insurance coverage and any billing and payment issues you may have.

The office is located at: 8166 Douglas Rd., Lambertville, MI, 48144, and is open from 8:00 a.m. to 4:00 p.m., Monday through Friday, except holidays.

Financial Assistance

Transportation:
The Monroe Cancer Connection at (734) 241-8888 can provide information. Free transportation is offered both locally and out of the county to and/or from treatments and appointments.
Handicapped Parking Placards:
Applications for Ohio are available.  Contact your Patient Advocate at (734) 847-4900 X135. Applications for Michigan can be obtained at any Bureau of Motor Vehicles.
Prescription Assistance:
Prescription Assistance is available for patients who have financial difficulties purchasing prescriptions. For more information call (734) 847-4900 X135.
Medical Supplies:
For information regarding the following free medical supplies:  Ostomy Supplies; Mastectomy Supplies; Wound Care Supplies; Adult Briefs; Wigs/Hats/Turbans. Contact the Monroe Cancer Connection at (734) 241-8888
Durable Medical Equipment:
For information regarding the following loan closet items: Bath Seats; Wheel Chairs; Crutches/Canes; Bedside Commodes; Hospital Beds; Walkers.  Contact the Monroe Cancer Connection at (734) 241-8888.
Nutritional Supplements:
For information on obtaining Ensure products at discounted prices.  Contact the Monroe Cancer Connection at (734) 241-8888.
Utilities Assistance:
The Home Energy Assistance Program (HEAP) is a federally funded program designed to help eligible low income Ohioans meet the high costs of home heating. For information call HEAP toll free (8:00 am to 5:00 pm).  Monday - Friday at (800)282-0880.
Financial Assistance:
Assistance is available for Co-pay, Deductible, Max Out-of-pocket expenses for patients with Chemo induced anemia and/or neutropenia. For information please contact the patient advocate at (734)847-4900 X135.
